CPC需要包含7個(gè)要素
a. Identification of the product covered by this certificate: 產(chǎn)品識(shí)別，可以寫產(chǎn)品名稱型號(hào)等
b. Citation to each CPSC product safety regulation to which this product is being certified:產(chǎn)品符合的美國法規(guī)或者標(biāo)準(zhǔn)，產(chǎn)品不同可能適用的法規(guī)標(biāo)準(zhǔn)會(huì)有差異，此處需要根據(jù)實(shí)際產(chǎn)品的測(cè)試報(bào)告填寫
c. Identification of the importer or domestic manufacturer certifying compliance of the product:美國進(jìn)口商或者美國國內(nèi)制造商
d. Contact information for the individual maintaining records of test results:測(cè)試結(jié)果維護(hù)人的聯(lián)系方式，一般寫測(cè)試申請(qǐng)商的聯(lián)系方式
e. Date and place where this product was manufactured:產(chǎn)品何時(shí)何地生產(chǎn)的，日期寫到月份即可；地址寫到城市即可
f. Date and place where this product was tested for compliance with the regulation(s) cited above:產(chǎn)品測(cè)試的時(shí)間和地點(diǎn)
